I, the undersigned, being the Parent/Legal Guardian of the student(s) declare that the information given in this application is correct and hereby apply for a place for my child at Theatricool Summer School. I understand that Theatricool reserves the right to restrict admission at its own discretion.
The Personal Data included in this form will be used by Theatricool for the purposes of administration, research, the provision of teaching services in singing, dance and drama, the organisation of performances and for the administration of Theatricool's statutory obligations under legislation relating to children. Theatricool will not disclose this information to third parties for marketing purposes.
